No. Minnesota is a direct-access state, therefore, you do not need a referral to receive treatment from a physical therapist. If you plan to use out-of-network benefits, we encourage you to contact your insurance company prior to your first visit to determine if a referral or pre-approval is required.

No. We are out-of-network with all commercial insurance plans, including Medicare and Medicaid. You can call your insurance and ask if you have “out-of-network benefits”. We can provide you with a superbill that you can submit to your insurance. They will then reimburse you directly according to your coverage.

Because Lumina Physical Therapy does not bill insurance, we are able to provide preventative, wellness, performance enhancing services and long term maintenance that is not ‘medically necessary’. Your insurance company no longer calls the shots- you do!
